- An up-to-date browser or the secure visit app provided in advance

- Client identity verified at the start of each visit
- Documentation stored in our secure medical record
- No recording without your written authorization
Your responsibilities
- Be in the state where you are licensed to receive care at the time of visit.
- Provide accurate health information and a current medication list.
- Notify us before the visit if a third party will join.
- Use a secure, private location and personal device when possible.
Telehealth Consent
What you're agreeing to.
By participating in a telehealth visit with Tether Wellness, you acknowledge and agree to the following:
- Benefits include increased access, reduced travel, and continuity of care.
- Risks include possible technology interruptions, limitations of remote examination, and the possibility that a remote visit may not be sufficient and an in-person visit may be required.
- Privacy is protected through HIPAA-aligned platforms, but no electronic transmission can be guaranteed fully secure.
- State licensure: Clinicians may only provide care to clients physically located in states where they are licensed. You agree to disclose your location at the time of the visit. Services are generally limited to residents of Florida unless otherwise indicated.
- Emergencies: Telehealth is not appropriate for emergencies. You agree to call 911 or seek immediate in-person care when needed.
- Documentation: Visits are documented in your medical record; no recording is made without your written authorization.
- You may withdraw consent for telehealth at any time and request in-person care instead.
