Our Responsibilities

  • For payment, billing, claims, and authorizations for the services you receive.
  • For health care operations, quality improvement, staff training, and credentialing.

Your Rights

  • Inspect and obtain a copy of your records.
  • Request an amendment to your records.
  • Request restrictions on certain uses or disclosures.
  • Request confidential communications.
  • Receive an accounting of disclosures.
  • Be notified of a breach of unsecured PHI.
  • File a complaint without retaliation.

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at hello@tetherwellness.com or with the U.S. Department of Health and Human Services.
